Hospice Care Liaison

Ohio's Hospice LifeCare is seeking a dedicated and compassionate RN/LPN Admission Care Liaison to join our team. As a member of our team, you will have the opportunity to make a meaningful impact in the lives of our patients and their families.
The RN/LPN Admission Care Liaison plays a vital role in ensuring that our patients receive quality nursing care and coordination of hospice services. This position requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:
- Admit patients into the hospice program efficiently and provide them with the necessary resources to support their end-of-life journey.
- Maintain accurate and timely documentation in the electronic medical record.
- Meet or exceed a conversion rate of 75%.
Requirements:
- Active Registered Nurse license or LPN license in the State of Ohio without any board actions.
- Two years of acute care nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
- CPR Certified.
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime, or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance Coverage.
