  • Assist patients with daily living activities, such as meal preparation and medication management
  • Monitor patients' vital signs and report any changes to the healthcare team
  • Document patient care and health information accurately and timely
  • Communicate effectively with patients, families, and healthcare professionals
Requirements
  • Nurse Aide certification in good standing with the Wisconsin Nurse Aide Registry
  • Basic Life Support (BLS) certification for Healthcare Providers
  • Valid driver's license
  • 1 year of experience in home health care or a related field
Preferred Qualifications
  • Experience working with elderly, convalescent, or disabled patients
  • Knowledge of home health care principles and practices
  • Excellent communication and interpersonal skills
Working Conditions

This position requires frequent travel to patients' homes, which may involve exposure to severe weather and other adverse conditions. The caregiver must be able to lift up to 50 pounds, push/pull with 30 pounds of force, and perform a sliding transfer of 150 pounds with a second person present.
