Job Summary:

As a Home Health Care Rehabilitation Lead at Advocate Health, you will play a vital role in providing exceptional patient care and leading a team of rehabilitation therapists. This is an exciting opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

Acts as a resource to employees, providing training, day-to-day work direction, and input on performance.
Assigns, monitors, and reviews progress and accuracy of work, directing efforts and providing guidance on complex issues.
Assists with human resource responsibilities, including interviewing and selection of new employees, staff development, resolution of employee concerns, and employee morale.
Manages caseloads of staff, evaluating patients, ensuring compliance with established Care Plans, and performing Joint Visits.
Follows all State of Wisconsin and Federal Regulations pertaining to the supervision and role of Physical Therapist Assistants in Homecare.

Requirements:

Physical Therapist license issued by the state of Wisconsin, or Occupational Therapist license issued by the state of Wisconsin.
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) within 6 months.
Valid drivers license issued by the Division of Motor Vehicles, or Driver license issued by the state of Illinois.
Bachelor's Degree in Physical Therapy, or Bachelor's Degree in Occupational Therapy.
Typically requires 5 years of experience in physical therapy or occupational therapy, including experience within home health care.

Preferred Qualifications:

Proficiency in clinical and organizational skills with the ability to work independently and make sound judgments.
Effective interpersonal and communication skills.
Must possess and provide proof of a valid Driver License and a registered, functional car with adequate insurance coverage.

Working Conditions:

Must be able to work safely with people of various size, weight, diverse and complex medical conditions, and physical limitations.
Must be able to sit, stand, walk, and climb stairs, gain entry to patient's residence, travel from and to assigned visits, and attend to a variety of work-related activities.
Must have the physical agility to move about in confined spaces, including bending, twisting, kneeling, squatting, and occasionally reaching one or both overhead.
Must be able to lift up to 50 lbs. from floor to waist, lift up to 20 lbs. over the head, carry up to 40 lbs. a reasonable distance, push/pull with 30 lbs. of force, and perform a sliding transfer of 150 lbs. with a second person present.
Must be capable of the manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements, and office equipment.
Must have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest.
Must be able to concentrate with frequent interruptions and remember multiple tasks and functions over a period of time.
Must have accurate vision to read gauges and measuring devices and to interpret written instructions.
Operates all equipment necessary to perform the job.
