Job Context:
The Licensed Practical Nurse (LPN) plays a crucial role in collaborating with the healthcare team and patients to evaluate, plan, facilitate, and advocate for services that address patients' health requirements, ultimately promoting quality and cost-effective outcomes.

Responsibilities:
Conduct comprehensive assessments of patients' health and psychosocial needs, developing collaborative treatment plans with patients and the healthcare team. Complete necessary referrals and promote patient empowerment and self-management goals. Monitor, evaluate, and document patient progress while collaborating with external service providers to coordinate treatment.
Record all interventions, whether through phone, email, or in-person, in patient charts and compile reports on service delivery and outcomes. Maintain timely correspondence through various communication methods.
Work closely with senior leadership to obtain data for outcome measurement reporting and ensure continuity of care by communicating relevant information across departments.

Essential:
- Oklahoma Licensed Professional Nurse (LPN) license required

Physical Requirements:
Regularly required to sit, walk, and stand; talk and hear, both in person and by telephone. Must have the strength to assist with lifting and transferring patients and perform CPR. Requires mobility to bend, stoop, and navigate confined spaces.
