**Job Title:** Health Home Coordinator

**Job Summary:** We are seeking a highly skilled and compassionate Health Home Coordinator to join our team at Andwell Health Partners. As a Health Home Coordinator, you will play a critical role in providing comprehensive care to individuals with co-occurring mental health and substance use disorders.

Key Responsibilities
  • Conduct face-to-face comprehensive assessments and develop initial plans of care within 30 days of enrollment, and reassess annually or as needed.
  • Maintain accurate and timely case records, ensuring compliance with regulatory requirements.
  • Collaborate with healthcare providers, community partners, and family members to develop and implement individualized care plans.
  • Refer patients to necessary medical services, including dental care, palliative care, hospice, and home health.
Requirements
  • Bachelor's degree in a related field, such as psychology, social work, or human services, or a bachelor's degree in an unrelated field with at least 1 year of full-time experience in human services.
  • Minimum of 2 years of experience in the behavioral health field, with a focus on case management and care coordination.
  • Valid driver's license and reliable transportation.
