About the RoleThis is a full-time position located at our center in Middle Tennessee. As a Family-Centered Coach, you will work directly with participants to help them identify and achieve their goals. You will use Family Centered Coaching principles to serve the whole family, keeping them in the driver's seat and centering them as experts on their own strengths and challenges.
Key Responsibilities- Lead families in coaching and care coordination, building a team of support that guides them towards their self-identified goals.
- Connect families with needed community resources, including services from our partner organizations and TAEM's Navigators and Specialists.
- Provide holistic services to caregivers, including intake assessments, goal setting, long-term action plan development, progress monitoring, advocacy, and referrals.
- Maintain regular communication with families, conducting home visits as needed and meeting with them for 18-36 months on average.
- Main duties also include maintaining 25 enrolled families at a given time, inputting data into our system, and using data to inform decisions.
We offer a competitive salary range of $45,000 - $55,000 per year, depending on experience. This estimate includes potential bonuses based on performance and qualifications.
