Job Description:

Critical Access Hospital Pediatric and Adult: Provide high-quality nursing care to patients in a safe, efficient, and courteous manner, utilizing evidence-based practices and proper nursing procedures.

Support patients and families during the dying process, and perform admission assessments and subsequent evaluations. Complete required documentation, address patient learning needs, and develop individualized care plans.

Enter orders via computer, participate in daily unit operations, and attend required in-service, annual education, and departmental meetings. Contribute to quality improvement initiatives, adhere to hospital and departmental policies, and maintain effective communication with colleagues, physicians, and other healthcare professionals.

The ideal candidate will possess knowledge of growth and development principles across the lifespan, and be able to assess patient data to identify age-specific needs. They will also be able to interpret relevant information to provide care that meets unit, area, and departmental policies and procedures.

Essential Functions:
  • Conduct thorough head-to-toe physical assessments and social assessments upon admission.
  • Develop and update written care plans daily.
  • Initiate cardiac monitoring, oxygen therapy, and other interventions as needed.
  • Deliver nursing care through the nursing process: assessment, problem identification, planning, interventions, and evaluation.
  • Reassess patients to identify progress and trends requiring intervention or reporting to physicians.

Assist patients, physicians, and peers in determining long- and short-term goals, and formulate plans to achieve those goals. Assist with telemetry monitoring, maintain knowledge of basic arrhythmia interpretation, and operate and manage equipment such as IV pumps and telemetry monitors.

Administer medications, calculate doses, and maintain current knowledge of medications. Maintain patient status awareness, work with the healthcare team to adjust and improve recovery approaches, and utilize critical thinking skills for appropriate patient interventions.

Document medical and nursing treatment, nursing care, interventions, and outcomes in a timely and accurate manner. Complete documentation necessary for transition between statuses, including to swing bed. Assist physicians with patient care, make rounds, and provide progress reports on patient status.

Follow procedure for medication administration and narcotics control, utilize EMAR, scans, and use two sources of patient ID before all medications and procedures. Assume professional responsibility for ensuring patient care meets the highest standards possible.

Communicate questions about medical or nursing care through the chain of command, as needed, utilizing the SBAR form of communication. Ensure consents are obtained when needed, and that all records are signed and legally complete. Ensure completion of time-out procedures as appropriate.

Complete incident and error forms when necessary. Assess patient learning needs, educate patients and families, and treat patients and their families with respect and dignity.

Practice within Nursing Standards of Practice and follow standards of patient care and CRH Policies and Procedures. Adhere to MD orders.
