MDS Coordinator Job Description

Laurelwood Health Care Center is seeking a qualified MDS Coordinator to join our team.

Job Summary

The MDS Coordinator will be responsible for coordinating the facility's Resident Assessment Instrument (RAI) process in accordance with state and federal regulations. This includes accurate completion of all MDS assessments and any supporting assessments or clinical documentation.

Key Responsibilities
  • Coordinate the RAI process and ensure compliance with state and federal regulations.
  • Accurately complete all MDS assessments and supporting documentation.
  • Implement and evaluate each resident's comprehensive plan of care.
  • Schedule and lead interdisciplinary care plan team meetings.
  • Audit medical records for supporting documentation.
  • Provide education to department heads, physicians, and other staff as needed.
  • Collaborate with health plans, physicians, and the interdisciplinary team for insurance approvals and eligibility.
  • Interpret rules, regulations, and coverage guidelines.
  • Utilize the RAI Manual as a resource during the assessment coding process.
  • Schedule ARDs for payment assessments and transmit assessments in accordance with current regulations.
  • Confirm transmission files and ensure the Business Office designee receives validation reports.
  • Facilitate the completion and updating of resident care plans with the IDT.
  • Attend continuing education related to the RAI process and related programs.
  • Complete all assigned training and skills competency.
  • Perform administrative duties as assigned.
