Key Responsibilities:
- Develop and implement effective discharge plans for patients with renal dialysis needs.
- Collaborate with the interdisciplinary team to identify and address barriers to discharge.
- Provide education and support to patients and their families to ensure a smooth transition to post-acute care.
- Monitor patient progress and adjust discharge plans as needed.
Requirements:
- Master's degree in Social Work or related field.
- Current/Valid license in the State of Maryland; LCSW-C/LCPC required.
- Minimum of 3 years of experience in social work, with a focus on renal dialysis or complex care.
