About the Position:

We are seeking a highly skilled Healthcare Professional - Case Management and Community Resources to join our team at Trinity Health Corporation. In this role, you will play a vital part in coordinating patient care and connecting patients with essential community resources.

Key Responsibilities:

  • Assess patient needs and identify opportunities for referral to community resources and rehabilitation services.
  • Collaborate with interdisciplinary teams to coordinate patient care, including referrals to home care, hospice, and other community-based services.
  • Provide education and support to patients and families on healthcare options, benefits, and available resources.
  • Connect patients with local resources, including non-profit organizations, government agencies, and faith-based initiatives.

Requirements:

  • Bachelor's degree in Social Work or related field required; Master's degree preferred.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the state of Connecticut.
  • Certification in case management (CCM) and Continuity of Care (ACCC) desirable.
  • Excellent communication and interpersonal skills, with ability to work effectively in a fast-paced environment.
