We are seeking a compassionate Social Worker to support families and caregivers through bereavement. This role involves conducting assessments, developing bereavement care plans, and providing counseling to address psychosocial needs and support grieving families.

Key Responsibilities
  • Conduct initial bereavement assessments and develop care plans within 24 hours.
  • Provide individual and group counseling and assist with understanding grief and loss.
  • Maintain communication with the Hospice Admissions Team, Volunteer Department, and healthcare professionals.
  • Prepare and maintain accurate documentation and reports.
  • Review financial status of bereaved and assist in finding financial support.
  • Facilitate bereavement support groups and provide staff/community education.
  • Participate in on-call rotation and provide emergency coverage as needed.
Qualifications
  • Master's degree in social work (MSW) from an accredited institution.
  • Licensed Clinical Social Worker (LCSW) in Florida required.
  • Three years of social work experience; hospice experience preferred.
  • Valid Florida driver's license, auto insurance, and reliable transportation.
  • Ability to work independently, communicate effectively, and use office software.
