Skilled Nursing Care Specialist

Job Summary:

As a Skilled Nursing Care Specialist, you will be responsible for providing skilled nursing care to residents according to established care plans and in compliance with federal and state regulations. You will work closely with our interdisciplinary team to ensure the coordination of care and provide guidance and direction to ancillary staff.

Key Responsibilities:
  • Provide quality care to acute, subacute, and chronically ill residents, administering medications and treatments according to standards of care, and maintaining competency in all required nursing skills.
  • Provide direction to CNAs and monitor the workflow of the unit to ensure coordination of care, using the nursing process for assessment, nursing diagnosis, planning, intervention, and evaluation.
  • Maintain accurate inventory of medications at the beginning and end of each shift, ensuring accurate distribution of medications to all residents according to the 5 rights, monitoring for medication side effects, and ensuring that medications are provided in a manner consistent with manufacturer's guidelines.
  • Give guidance and direction to auxiliary staff, participating in report meetings at the beginning and end of each shift, communicating with residents, guardians, and family members on the current condition and needs of residents in accordance with regulations and standards of practice, providing teaching and training to residents and families as needed.
  • Ensure that preventative measures are in place, such as specialty mattresses, foam boots, weekly skin checks, turning/repositioning schedules, toileting schedules, and cushions in all wheelchairs, following up on reports of rashes, open areas, edema, blisters, skin tears, or bleeding.
  • Respond promptly to alarms, ensuring that restraint alternatives are in place/activated, including personal alarms, cushion alarms, wheelchair belts, tumble mats, alarm mats, bolstered beds, hip protectors.
  • Provide and ensure quality end-of-life care, using comfort baskets to meet resident and family needs, ensuring hygiene, comfort, and spiritual needs are met, treating pain/restlessness, offering family courtesy meal trays and other amenities, demonstrating compliance with post-mortem care procedures, and participating in departure ceremonies.
Requirements:
  • Graduate of an accredited school of nursing.
  • No experience required.
