Job Title: Care Coordinator

At InterCare Health Network, we are seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in assisting patients and their families in managing their healthcare needs.

Key Responsibilities:
  • Collaborate with care managers to provide comprehensive care to patients with diverse needs
  • Identify and address social determinants of health, such as food resources, transportation, and housing concerns
  • Conduct transitions of care and provide referrals to community agencies and patient advocacy services
  • Provide care coordination, coaching, and health education through ongoing follow-up and motivational interviewing
  • Ensure patients understand their care plans and instructions
Requirements:
  • High school diploma or equivalent
  • 126 hours of Community Health Worker Michigan-specific training
  • 4 hours of MI Bridges Medicaid enrollment training certification
  • Certified Application Counselor (CAC) curriculum completion within 6 months
  • BCLS certification within 90 days
