Clinical Operations Manager

CenterWell Home Health is seeking a skilled Clinical Manager to lead our clinical operations and ensure the delivery of high-quality patient care. As a Clinical Manager, you will be responsible for overseeing the development, implementation, and analysis of clinical operations for our locations. This is a challenging and rewarding role that requires strong leadership and clinical expertise.
Key Responsibilities- Develop and implement clinical operations plans to ensure efficient and effective delivery of patient care.
- Conduct regular assessments and reassessments of patients to ensure their needs are met and care plans are updated accordingly.
- Manage the assignment of caregivers and ensure they have the necessary skills and training to provide high-quality care.
- Oversee the delivery of care to all patients served by the location, ensuring compliance with regulatory agencies and Company policies.
- Collaborate with clinicians to establish immediate and long-term therapeutic goals, set priorities, and develop patient Plans of Care (POCs).
- Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers to ensure the appropriateness of care and outcome planning.
- Carry out supervisory responsibilities in accordance with Company policies and procedures.
- Participate in the interviewing, hiring, training, and development of direct care clinicians.
- Assess staff education needs and create and conduct regular staff education as needed.
- Graduate of an accredited School of Nursing.
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
