We are seeking a skilled and compassionate Patient Care Specialist to join our team at Alameda Health System. As a key member of our nursing staff, you will provide high-quality patient care and support to our patients and their families.
Key Responsibilities- Provide Patient Care: Accompany, assist, and represent the needs of patients to other healthcare providers.
- Administer Medications and Treatments: Accurately administer medications and treatments, and provide care with respect to skin and wound care, ADLs, and other essential patient care activities.
- Develop and Implement Care Plans: Develop, implement, evaluate, and modify nursing care plans to meet the unique needs of each patient.
- Collaborate with Healthcare Team: Gather and assess information about patients, assist and consult with physicians, and contract with other departments to obtain or provide patient information.
- Supervise and Train Staff: Organize, supervise, and make adjustments to the quality of care provided by co-workers, and assume responsibility as a team leader or charge nurse when assigned.
- Promote a Healthy Environment: Participate in promoting a healthful, safe, and therapeutic environment for patients and families, and set up and control the environment essential for infection control.
- Participate in Quality Improvement: Participate in the implementation of patient care standards, infection control standards, and quality assurance criteria, and assist with conducting studies and participating in unit and other meetings.
- Education: Graduate of an accredited Nursing Program required; Bachelor's degree in Nursing preferred.
- Experience: Six months experience as a Clinical Nurse I with AHS or the equivalent of one year full-time recent experience at a comparable level in a minimum 100-bed hospital or in the area of specialty.
- Licenses/Certifications: Valid license to practice as a Registered Nurse in the State of California, and other certifications as required by the facility or unit/department specialty.
