**Job Summary:**

We are seeking a skilled Registered Nurse to join our team as an Elderly Care Specialist. In this role, you will provide care management and coordination for at-risk frail elder patients with complex medical and psychosocial issues.

As a core member of the patient-centered PACE interdisciplinary team (IDT), your goals will be to assure a systematic approach to the coordination of health services for patients along the care continuum. You will assist providers in identifying needs and coordinating access to appropriate care and services to improve or maintain the patient's functional status and independence.

This is a full-time position that requires travel between multiple locations. The ideal candidate will have relevant training and experience in gerontological nursing and be able to work effectively in a collaborative team environment.

**Key Responsibilities:**

  • Care management and coordination for at-risk frail elder patients
  • Identify needs and coordinate access to appropriate care and services
  • Assist providers in improving or maintaining patient functional status and independence
  • Collaborate with the IDT to develop and implement comprehensive care plans

**Required Qualifications:**

  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager

The estimated annual salary for this role is approximately $83,841-$131,391, based on a 36-hour workweek and assuming an average hourly wage within the listed range.
