Elder Care Specialist

We are seeking a compassionate and skilled Elder Care Specialist to join our team at Providence PACE in Beaverton-Hillsboro, OR.
About the RoleThis is a per diem/on call position supporting our site and surrounding areas as needed. As an Elder Care Specialist, you will provide care management, coordination, skilled care, and services for at-risk frail elder patients with complex medical and psychosocial issues.
About UsProvidence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults healthy and independent in their communities through clinics, home visits, and other services.
Responsibilities- Provide comprehensive nursing care and services to patients in their homes or in our facilities.
- Coordinate with healthcare providers and community resources to ensure seamless care transitions.
- Develop and implement individualized care plans to meet patients' unique needs and goals.
- Maintain accurate records and reports, including patient charts and billing information.
- Collaborate with our interdisciplinary team to achieve optimal patient outcomes.
- Bachelor's Degree in Nursing or related field.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Relevant certifications applicable to the position, as specified by the Nurse Manager.
The estimated salary range for this role is $40.20 - $63.64 per hour, depending on experience. Additionally, we offer a comprehensive benefits package, including health insurance, retirement savings plan, paid time off, and more.
