Nurse Case Manager I/II

The Case Manager I/II Emergency Department has a deep understanding of emergency care, disease processes, and utilization management. This position is responsible for ensuring appropriate ED utilization, placing patients in the appropriate status, and meeting patient care coordination needs and mitigating barriers to care. All nursing practice is based on the legal scope of practice, national and specialty standards, Children's National Policies and Procedures, and in accordance with all applicable laws and regulations.
Responsibilities- Assess patients' care management and coordination needs
- Develop and implement care plans to meet patients' needs
- Partner with physician practices and the care management team to coordinate patient care
- Work with payers and community resources to develop and facilitate effective, efficient care delivery options for patients
Minimum Education: Bachelor's Degree Plus Associate's Degree in Nursing if no BSN (Required)
BSN (Preferred)
Minimum Work Experience: 5 years Related experience. (Required)
