Hospice Physician

We are seeking a compassionate and dedicated Hospice Physician to join our team at Our Hospice of South Central Indiana. As a Hospice Physician, you will play a vital role in providing high-quality medical care to our patients and their families.
Key Responsibilities:- Provide medical direction and oversight of care plans and treatment for hospice patients
- Comply with state and federal guidelines, and document related to hospice process
- Make quality decisions regarding medical treatment and care
- Work collaboratively with the Interdisciplinary Group (IDG) to make decisions regarding admissions to hospice home care or inpatient settings
- Participate in development of individual plans of care before admission of a patient/family to hospice program
- Act as primary/attending physician when patients have no primary/attending physician
- Participate in medication review, titration, or discontinuation, in line with patient's condition and goals
- Rotate for weekend and holiday call to provide for the 24 hour, 7 day a week needs for hospice in all levels of care
- Visit hospice patients as indicated, including making visits to hospice patients at their home or a residential facility for symptom management or re-certification of hospice eligibility
- Doctor of Medicine or Doctor of Osteopathy Degree
- Experience in a medical specialty dealing with terminally ill patients, such as internal medicine, family medicine, oncology or geriatrics
- Demonstrated experience in the principles of pain and symptom management
- Hospice and Palliative Medicine board certification preferred
- Licensed Physician in the state of Indiana
- Valid Indiana driver's license and current auto insurance
- Current and updated DEA license
- Obtain and maintain privileges at Columbus Regional Hospital
- Remain current in CME hours in a primary specialty
