Primary Care Physician

As a Primary Care Physician at ChenMed, you will play a crucial role in delivering exceptional healthcare to our senior patients. Your responsibilities will include:
Patient Care:
- Independently manage care for patients with both acute and chronic health conditions, focusing on the older adult population.
- Build trusting relationships with patients to effectively influence their health outcomes and encourage positive behavior changes.
- Serve as a health coach, assisting patients in setting and achieving their health goals.
Collaboration:
- Work closely with management and medical directors to ensure adherence to clinical guidelines and participate in quality management initiatives.
- Engage with hospitalists to coordinate care for patients during hospital stays.
Clinical Responsibilities:
- Assess, diagnose, document, and treat patients with complex health needs, promoting education and coordinated care.
- Maintain compliance with departmental goals, regulatory standards, and quality patient care policies.
- Deliver consistent patient care experiences across various platforms, including in-person visits, telehealth, and home visits.
Professional Development:
- Stay updated on evidence-based practices and lifestyle interventions that impact patient health outcomes.
- Collaborate with care teams to achieve high-quality results and quality measures.
- Prioritize physician wellness to sustain a healthy work environment for staff and patients.
Qualifications:
- MD or DO in Family Medicine, Internal Medicine, Geriatrics, or a related specialty.
- Active State Medical License required.
- Bilingual in English and Spanish is essential.
- Board certification in relevant specialties is preferred.
