As a Senior Primary Care Physician at ChenMed, you will be responsible for delivering exceptional clinical care and ensuring cost-effective outcomes for a panel of up to 550 patients. Your role will focus on fostering trusting relationships with patients, guiding them in achieving their health objectives, and acting as a health coach rather than merely a consultant.
ChenMed is committed to revolutionizing healthcare for Medicare Advantage seniors by providing high-quality, PCP-led care tailored to their needs. Our approach is rooted in a proven care delivery model that emphasizes seamless, digital-first healthcare experiences.
Key Responsibilities:
- Provide comprehensive care for patients with both acute and chronic conditions, particularly in the older adult demographic.
- Take full responsibility for patient outcomes and engage specialists as necessary, maintaining active involvement in patient care.
- Collaborate with management and medical directors to adhere to clinical guidelines and participate in quality management initiatives.
- Engage with hospitalists to ensure continuity of care for hospitalized patients.
- Evaluate, diagnose, document, and manage complex health needs while promoting patient education and coordinated care.
- Comply with departmental objectives, regulatory standards, and quality care protocols.
- Lead a multidisciplinary care team to deliver exceptional patient experiences and meet performance metrics.
- Provide consistent patient care across various settings, including in-center visits, home visits, and telehealth.
- Contribute to center management and assist in patient recruitment efforts.
- Foster a positive workplace culture centered on accountability, compassion, and dedication.
- Perform additional duties as assigned by management.
Qualifications:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required.
- Active State Medical License is mandatory.
- Board certification in the relevant specialty is preferred; Board Eligibility is required.
- Basic Life Support (BLS) certification is essential.
- Possession of a current DEA number for controlled substances is required.
