About the Role

The Detroit Medical Center is seeking a highly skilled Patient Care Navigator to join our team of healthcare professionals in Detroit, Michigan.

As a Patient Care Navigator, you will play a vital role in facilitating patient care along a continuum through effective resource coordination to help patients achieve optimal health and access to care.

This position requires strong communication and organizational skills to ensure seamless transitions between care settings and promote timely throughput, safe discharge, and prevent avoidable readmissions.

Responsibilities
  • Care Coordination: Collaborate with multidisciplinary teams to develop and implement comprehensive care plans that address patient needs and preferences.
  • Transition Management: Identify and address potential barriers to transition, ensuring smooth handovers between care settings and minimizing delays.
  • Utilization Management: Conduct medical necessity screenings and submit for physician advisor review to optimize resource utilization and prevent unnecessary admissions.
Requirements
  • A minimum of 2 years of experience in case management or a related field.
  • Strong understanding of healthcare regulations and standards, including TJC accreditation requirements.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
