Job Summary

We are seeking an experienced Attending Physician to join our team at Theoria Medical. As a key member of our post-acute care team, you will provide high-quality, patient-centered care to residents in skilled nursing facilities across the country.

Responsibilities
  • Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care
  • Provide comprehensive primary care services, including diagnosing and treating acute and chronic illnesses
  • Document patient care in our proprietary electronic health record system
  • Participate in quality initiatives and programs, including Chronic Care Management and Transitional Care
  • Supervise collaborating nurse practitioners and physician assistants
Requirements
  • Board certification in a relevant medical specialty
  • Experience working in post-acute care settings
  • Strong communication and collaboration skills
  • Ability to work independently and as part of a team
