As a member of our interdisciplinary care team, you will play a vital role in delivering high-quality patient care and promoting a positive work environment.
Responsibilities- Provide professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician.
- Assess residents' physical and psychosocial needs, develop and implement individualized care plans, and provide education to residents and their families.
- Administer medications and treatments as ordered, and document resident interactions and outcomes.
- Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives and contribute to a culture of excellence in patient care.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse or Associate degree in nursing as a Registered Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) or Registered Nurse (RN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department-specific competencies and certifications.
