Abdominal Transplant Nurse Coordinator

We are seeking a skilled and compassionate RN to join our Abdominal Transplant team as a Transplant Coordinator. In this role, you will play a critical part in ensuring the smooth transition of patients through the transplant process.
Key Responsibilities- Assess patient needs and create personalized care plans to ensure optimal outcomes
- Coordinate tests and evaluations for transplant candidates, identifying any issues and arranging necessary care
- Manage recipient post-transplant care, including arranging admissions, lab tests, and clinic visits
- Teach patients, families, and caregivers about the transplant process, including evaluation, organ allocation, waiting, post-operative, and immunosuppression effects
- Use insurance knowledge to assist with post-transplant prescriptions and address coverage issues
- Comply with regulatory policies and maintain records on the transplant waitlist through various online programs
- Associate's degree in nursing
- Two years of professional experience as a RN
- Basic Life Support skills required or obtain BLS certification within 30 days if not already held
- Knowledge of medical terms, health practices, disease control, medication specs, and side effects
- Excellent communication skills, both spoken and written
- Basic computer skills, including Microsoft Office, information security, scheduling systems, payroll systems, electronic medical records, and email use
