As a key member of our healthcare team, you will be responsible for coordinating the planning, development, and implementation of comprehensive care plans that prioritize quality, dignity, compassion, and confidentiality for each individual.

Key Responsibilities:
  • Coordinate the planning, development, and implementation of a comprehensive care plan that addresses the holistic needs of patients.
  • Demonstrate critical thinking skills and problem-solving techniques to ensure optimal patient outcomes and unit goals.
  • Provide programs to support wellness and maintain flexibility in work schedule to connect regularly with off-shift staff.
  • Collaborate with the Assistant Director of Nursing (ADON) to monitor and assess workflow, delegate assignments, and develop procedures for care delivery using Evidence-Based Practice.
  • Supervise and investigate Reportable Events, such as medication errors and accidents/incidents, to ensure completion and follow-up.
  • Oversee accountability of the interdisciplinary team (IDT) for management of the unit and ensure patients receive value-based delivery of care.
  • Understand and follow Universal Precautions to prevent exposure to all body fluids and utilize techniques of infection control.
  • Provide monthly in-services to staff on the unit related to policies or timely clinical matters.
  • Ensure all significant changes in condition are reported to medical providers.
Requirements:
  • RN Licensure in good standing.
  • Prior Long-Term Care (LTC) experience.
  • Prior management experience.
  • BSN strongly preferred.
