We are seeking a highly motivated and experienced Wound Care RN to join our team at Chapel Manor in Philadelphia, PA.

About the Role

This is a full-time position that offers a competitive salary and a comprehensive benefits package. As a Wound Care RN, you will be responsible for leading a team of nurses and other healthcare professionals in the delivery of high-quality wound care services to patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities
  • Lead and integrate a fully developed Skin Health and Wound management program.
  • Use Evidenced-Based Guidelines to inform practice.
  • Own access to a robust formulary for all skin and wound interventions/treatment categories.
  • Collaborate with nursing center management and staff, clinical leadership, Wound Specialists, vendors, and others to effectively communicate with patients and their families regarding the prevention and management of skin and wounds.
  • Support central supply by serving as the clinical expert for skin, wound, and incontinent products and provide oversight for durable medical equipment related to the prevention and management of wounds.
Requirements
  • Must possess a RN license (MPT/DPT with leadership review/approval).
  • Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire.
  • Commitment to attend monthly support calls is required.
  • Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire.
  • Clinical experience in a skilled nursing setting is strongly preferred.
  • Discharge planning experience is strongly preferred.
  • Knowledge and experience with clinical charting, incident reporting, and investigation response is required.
