About the Role:

We are looking for a seasoned Geriatric Nurse Practitioner with experience in skilled nursing or long-term care settings. As a Geriatric Care Specialist, you will be responsible for providing comprehensive health assessments, collaborating with primary care physicians to create personalized care plans, and communicating effectively with patients and their families.

Key Responsibilities:

  • Conduct thorough health assessments to identify potential issues and develop effective care plans
  • Collaborate with primary care physicians to ensure seamless transitions of care
  • Communicate effectively with patients, families, and healthcare teams to ensure optimal outcomes
  • Participate in case rounds to review patient care and make recommendations for improvement

Requirements:

  • Current licensure as an Acute, Adult/Gerontological, or Family Nurse Practitioner in the state of practice
  • DEA licensure and CDS licensure (if required in the state of practice)
  • Basic Cardiac Life Support certification
  • 1 year of experience in skilled nursing or long-term care settings (preferred)
