Job Title: Clinical Supervisor - Care Coordinator

Advocates is seeking a highly skilled and experienced Clinical Supervisor - Care Coordinator to join our team. As a Clinical Supervisor - Care Coordinator, you will be responsible for providing supervision, oversight, and support to our Care Coordination staff within the Community Behavioral Health Center (CBHC). You will also conduct assessments and provide individual treatment to individuals served.

We are looking for a candidate who is passionate about working in a collaborative and team-oriented environment. You will have the opportunity to work with a diverse group of individuals and contribute to the development and management of our program.

Responsibilities:
  • Provide high-quality evaluations and individual clinical services.
  • Assist in treatment plan development in collaboration with the person served.
  • Provide focused leadership, support, and supervision to the care coordination staff.
  • Facilitate trainings and supervision for care coordination staff.
  • Develop and maintain deep knowledge of services offered by the agency, affiliate agencies, and community resources.
  • Provide administrative oversight of outpatient services to ensure that services and documentation meet regulatory standards and excellence in care.
  • Demonstrate a proactive commitment to maintaining effective communication with staff and customers to facilitate and promote effective working relationships and customer service.
  • Provide feedback to supervisees regarding clinical and documentation practices.
  • Provide weekly documented supervision to all supervisees.
  • Assist in the hiring and training of new care coordination staff.
  • Attend and actively participate in all supervisions and staff meetings.
  • In collaboration with the Supervisor of Clinical Services, contribute to the development and management of the program.
  • Attend trainings as assigned; maintain necessary certifications and licenses.
Qualifications:
  • Masters Degree in a related field required and independent licensure (LICSW, LMHC, LMFT) preferred.
  • Ability to communicate effectively verbally and in writing.
  • Strong computer knowledge.
  • Ability to thrive in a fast-paced, team-oriented environment.
  • Outstanding reasoning and problem-solving abilities.
  • Must have a valid driver's license and access to an insured vehicle.
  • High energy level, superior interpersonal skills, and ability to function in a team atmosphere.
  • Strongly prefer a candidate who will have a demonstrated understanding of and competence in serving culturally diverse populations.
