LPN Community Health Specialist

Midland Care is seeking a skilled LPN to provide nursing services in the home and clinic settings for our Senior Services programs.

Key Responsibilities:
  • Provide skilled nursing services to seniors in the home and clinic settings
  • Work collaboratively with interdisciplinary teams to deliver high-quality care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements:
  • Current unrestricted State of Kansas License to practice as a Licensed Practical Nurse
  • Valid Kansas Driver's License
  • At least one year of experience as an LPN, preferably in home or community health
  • Ability to relate well with seniors and their families
  • Experience working in an interdisciplinary team setting is desired
