Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Endeavors. As a Case Manager, you will be responsible for providing quality case management services to our service population, ensuring their needs are met and they receive the support they require to build better lives for themselves and their families.

Key Responsibilities
  • Coordinate referral, service planning, and documentation of services for assigned caseload.
  • Interview applicants to determine eligibility for program enrollment based on established guidelines and requirements.
  • Develop client intake and case plan for clients, instructing them in completing necessary paperwork, including goals and/or budget forms.
  • Meet regularly with clients to discuss and evaluate their progress, preparing accurate and up-to-date records documenting the same.
  • Plan, coordinate, and/or facilitate life management classes to assist clients in obtaining skills that will enable them to functionally cope with their environment.
  • Consistently serve as a mentor and provide positive examples of proper hygiene, personal appearance, records retention, budget planning, scheduling, etc.
  • Refer clients to appropriate programs or other agencies based on clients' needs and in accordance with their program plan.
  • Complete discharge plan of clients exiting program, transferring file contents to non-active file.
Requirements
  • 4-year bachelor's or undergraduate degree from an accredited program/university, preferably in one of the following areas: Social Work, Psychology, Criminal Justice, Sociology, or related field of study.
  • 1+ year related experience and/or training, including experience working in crisis intervention.
  • Proficiency in Spanish/English (written and spoken language skills), required.
  • Driver's License with clear record.
