Weekend Registered Nurse Position

We are seeking a compassionate Registered Nurse (RN) to join our team as a Weekend Float RN. As a Weekend Registered Nurse, you will work one-on-one with our patients in their home environment, establishing and overseeing all aspects of case-managing patient's care.
Responsibilities- Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.
- Collaborate with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
- Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
- Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
- Participate in on-call rotation as reasonably assigned.
- Minimum of 2 years nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
- Active MO & IL State Registered Nurse licensure is required.
- Proof of current CPR certification and valid Driver's license.
