About the Role

We are seeking an experienced Patient Navigation Specialist to join our team. In this role, you will be responsible for conducting initial assessments of new patients, coordinating their care plans, and organizing access to necessary healthcare services.

The ideal candidate will have excellent communication skills, strong attention to detail, and a passion for improving patient health outcomes.

Responsibilities:
  • Conduct Initial Assessments: Evaluate new patients' healthcare needs and develop personalized care plans.
  • Care Plan Coordination: Collaborate with healthcare professionals and community organizations to ensure seamless patient care.
  • Access to Healthcare Services: Coordinate appointments, diagnostic tests, and prescription medications to meet patient needs.
  • Electronic Medical Records (EMRs): Utilize EMRs to document patient information and track progress toward care goals.
  • Preventative Care Plans: Work with internal teams to develop and implement preventative care plans that improve patient outcomes.
  • Industry Knowledge: Participate in meetings and trainings to stay up-to-date on healthcare industry trends and best practices.
  • Insurance Coverage: Ensure insurance coverage for healthcare services and medications.
Requirements:
  • Bachelor's degree in social work or related healthcare field.
  • Minimum 2 years of experience in healthcare coordination, patient navigation, or related work.
  • Knowledge of healthcare delivery systems, protocols, and regulations.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management capabilities.
  • Ability to work well in a team-oriented environment.
  • Familiarity with EMR systems and other healthcare software programs.

Estimated Salary: $18.00 per hour
