Job Title: Registered Nurse - Home Health

Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver whole-person care.

Responsibilities:

  • Provide direct patient care, including assessments, medication management, and wound care
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Conduct home visits to assess patient needs and provide education on self-care and disease management
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Document patient progress and outcomes in accordance with regulatory requirements

Requirements:

  • Current RN licensure in the state of Oregon
  • 1 year of experience in medical-surgical or home health nursing
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to navigate electronic medical records and other healthcare software

Preferred Qualifications:

  • Experience in home health or hospice nursing
  • Geriatric or palliative care experience
  • ANCC Psychiatric-Mental Health certification
