Job Summary
We are seeking a skilled Private Duty LVN to provide high-acuity child care in New Caney, TX. This role involves implementing physician-prescribed plans of care, conducting patient assessments, and coordinating care with healthcare teams. The ideal candidate will have excellent communication skills, be able to work independently, and possess a strong understanding of pediatric nursing principles.

Responsibilities
* Implement physician-prescribed plans of care
* Conduct patient assessments and coordinate care with healthcare teams
* Administer prescribed medications, treatments, and therapies
* Compliantly document care provided in our electronic system
* Promote health education and training for family members
* Collaborate with Case Managers to ensure seamless care transitions

Requirements
* Active, unencumbered LPN license in the state of Texas
* Current CPR certification from American Red Cross or American Heart Association
* TB skin test (current within last 12 months)
* 6 months prior hands-on nursing experience preferred
* Reliable transportation required
