Job Description
This role involves working as part of a dynamic team to deliver high-quality care to a medically fragile child in New Caney, TX. As a Private Duty LVN, you will implement complex care plans, conduct thorough patient assessments, and communicate effectively with healthcare professionals. The ideal candidate will possess strong organizational skills, be adaptable to changing situations, and demonstrate a commitment to delivering exceptional patient outcomes.

Responsibilities
* Develop and implement comprehensive care plans based on physician orders
* Conduct thorough patient assessments and identify areas for improvement
* Coordinate care with multidisciplinary teams, including physicians, therapists, and Case Managers
* Administer prescribed medications, treatments, and therapies accurately and safely
* Maintain accurate and compliant documentation in our electronic system

Qualifications
* Active, unencumbered LPN license in the state of Texas
* Current CPR certification from American Red Cross or American Heart Association
* TB skin test (current within last 12 months)
* 6 months prior hands-on nursing experience preferred
* Reliable transportation required
