Position Summary
We are seeking an experienced Clinical Coordinator to join our team at the Hope Unity and Freedom Center. As a Clinical Coordinator, you will play a crucial role in the admission process for new members, interviewing, assessing, and creating personalized service plans tailored to their needs. Your clinical expertise and skills will help us make a positive impact on the lives of those we serve.
Key Responsibilities:
- Conduct comprehensive needs assessments for referred individuals.
- Develop and implement individualized person-centered service plans.
- Manage and respond to pending denials received from managed care organizations.
- Identify areas for professional growth and seek out training opportunities.
- Provide training to agency staff on specialized topics.
- Promote the program and maintain census and growth.
