Wound Care Coordinator Registered Nurse Job Description

Spring Creek Rehabilitation and Nursing Center is seeking a compassionate and enthusiastic Registered Nurse (RN) with Wound Care experience in long-term care to lead our nursing team.

Responsibilities:
  • Make comprehensive wound assessments and create plan of care including goals, implementation of treatment, evaluation and outcomes for residents throughout the Facility.
  • Provide residents, family and staff education regarding wound management, prevention of pressure ulcers, fistulas, and ostomies, support surfaces, dressing selections and other therapies.
  • Provide general direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel.
Requirements:
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse (RN) in this state.
  • Bachelors Degree in Nursing (BSN) from an accredited college or university preferred.
  • Two (2) years of experience preferred as a supervisor in a hospital, nursing care facility, or other related health care Facility.
