Hospice Social Worker PRN Position

Serenity Hospice is seeking a skilled Social Worker to join their team in Salem, OR. As a PRN Social Worker, you will play a vital role in providing compassionate care to patients and their families.
Responsibilities- Conduct thorough psychosocial assessments of patients and families to identify their needs and develop personalized plans of care.
- Counsel patients and their families to address stress, coping difficulties, and other emotional challenges.
- Provide accurate and timely documentation of patient services to ensure seamless care coordination.
- Collaborate with community agencies to secure resources and support for patients and their families.
- Assist patients and their families with applications for benefits and services.
- Master's Degree in Social Work (MSW) from an accredited program.
- Current State Licensure in Social Work (LMSW or LCSW).
- Minimum one year's experience in healthcare or hospice settings.
- Valid driver's license and proof of insurance.
