Hospice Social Worker PRN Position

Serenity Hospice is seeking a skilled Social Worker to join their team in Salem, OR. As a PRN Social Worker, you will play a vital role in providing compassionate care to patients and their families.
Responsibilities- Conduct thorough psychosocial assessments of patients and families/caregivers to identify their needs and develop a plan of care.
- Counsel patients and their families/caregivers to address stress and coping difficulties.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Assist patients and their families/caregivers in accessing community resources and benefits.
- Serve as a liaison between patients and families/caregivers and community agencies.
- Social Work Degree (Master's Degree in Social Work required in Oregon).
- Current State Licensure in Social Work (LMSW or LCSW in Oregon).
- Minimum of one year's experience in healthcare or hospice setting.
- Valid driver's license and proof of insurance.
- Flexible hours.
