We are seeking a skilled and compassionate Hospice Registered Nurse to join our team of dedicated professionals. As a key member of our home health team, you will be responsible for providing comprehensive care to patients with terminal illnesses, focusing on symptom management, pain control, and emotional support.

Job Summary

Our ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for delivering exceptional patient care. You will work closely with patients, families, and healthcare providers to develop and implement individualized care plans, ensuring that each individual receives the highest level of care and support.

Salary Information

The salary range for this position is $100-$200 per visit, reflecting your expertise and dedication to delivering exceptional patient care.

Responsibilities and Requirements

  • Assess physical, functional, psychosocial, and cognitive status of patients utilizing interview observations and physical exam techniques.
  • Develop and implement individualized care plans in consultation with patients, families, and healthcare providers.
  • Provide ongoing nursing assessment and care to patients and their families, addressing physiologic or psychosocial risks as needed.
  • Maintain accurate and detailed records of patient care, including assessments, interventions, and progress towards goals.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Current RN license in California.
  • Minimum 1 year experience as a professional nurse within the last three years, preferably in public health, home care, or hospice nursing.
  • Current CPR certification.
  • Licensed driver with an insured automobile.
