Key Responsibilities- Assessment and Treatment Planning: Conduct thorough assessments to identify individual strengths, needs, and barriers to achieving self-identified goals. Develop and implement personalized treatment plans to promote rehabilitation, recovery, and resiliency.
- Client Support and Education: Provide education and support to clients and their families on various topics, including symptom management, relapse prevention, and medication adherence. Teach skills classes to clients and encourage the development of self-motivational skills.
- Service Coordination: Assist clients in accessing necessary rehabilitative, medical, and other services. Coordinate services and resources to help clients gain access to community-based support systems.
- Crisis Intervention: Assess, support, and intervene in crisis situations, including the use of individual crisis management plans. Identify and encourage the use of alternatives to hospital emergency departments and inpatient hospital services.
- Documentation and Reporting: Maintain accurate and comprehensive client records. Complete and submit billing documentation as required. Maintain and report applicable statistics regarding programs and client services.
- Education: High School or GED Equivalent
- Experience: Essential: 1 year of experience in a mental health setting. Nonessential: Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
- Credentials: Essential: CPR for Healthcare/BLS Provider or Professional Rescuer within 30 days, Certified Peer Specialist recognized in the State of NM, Valid New Mexico Driver's License, UNM Vehicle Operator's Permit within 60 days
This position requires the ability to work in a fast-paced environment with frequent interruptions. The employee must be able to lift up to 50 pounds occasionally and 25 pounds frequently. The employee must also be able to work in a medical environment with exposure to dirt, dust, fumes, odors, and bad weather.
