Role Responsibilities
The Case Manager (CM) is responsible for ensuring optimal patient care by managing rehabilitative services. Key responsibilities include:
- Providing orientation to patients and families regarding the case manager's role, rehabilitation philosophy, and discharge criteria.
- Coordinating education for patients and families, promoting their involvement in care discussions.
- Implementing treatment plans that reflect the patient's needs and preferences.
- Documenting evaluations and communicating patient needs effectively.
- Fostering positive patient interactions to enhance service quality.
Qualifications Required
- Current RN licensure; BSN preferred.
- Minimum of 5 years nursing experience, including critical care or case management.
- Preferred certification in case management or utilization review.
- Experience with InterQual guidelines is advantageous.
- Basic Cardiac Life Support certification required within 30 days of employment.
- Associate Degree or Registered Nurse Diploma is necessary.
