Nurse Practitioner

We are seeking a skilled and compassionate Registered Nurse to join our team at Egan-Ochsner Home Health Northshore, a part of LHC Group, the preferred post-acute care partner for hospitals, physicians, and families nationwide.
About the Role:As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to achieve optimal health outcomes.
Responsibilities:- Initiate, develop, implement, and make necessary revisions to the plan of care in collaboration with the physician and other healthcare professionals.
- Directly and/or indirectly supervise care provided by home health aides and licensed practical vocational nurses, providing instruction as appropriate, and assigning tasks according to State and federal regulations.
- Provide high-quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care, and in coordination with other members of the healthcare team.
- Accurately and timely complete comprehensive assessments (OASIS) including medication reconciliation, and accurately and thoroughly document patient visits per policy, and payer requirements.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- At least one year of clinical experience as a Registered Nurse.
- Recent clinical experience as an LPN may be considered in lieu of one year of experience as a Registered Nurse.
