Key Responsibilities:- Wound Assessment and Documentation: Conduct thorough assessments of patients' wounds, documenting relevant information such as size, depth, and overall condition. Maintain accurate and detailed records of wound status, treatment plans, and progress notes.
- Wound Treatment and Dressing Changes: Administer prescribed wound care treatments following established protocols and physician orders. Perform dressing changes with precision, upholding aseptic technique and strict adherence to infection control standards.
- Patient Education: Educate patients and their families on wound care protocols, prevention strategies, and the importance of consistent adherence to treatment plans. Provide clear guidance on self-care for wounds, monitoring and ensuring patients' understanding and compliance.
- Collaboration with Healthcare Team: Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive wound care plans. Communicate effectively to facilitate seamless coordination of care and prompt adjustments to treatment strategies.
- Mobile Care Delivery: Travel to various locations, including patients' homes and healthcare facilities, to provide essential wound care services. Adapt to diverse environments and efficiently manage resources to deliver high-quality care in a mobile setting.
- Qualifications: Certification as a Licensed Practical Nurse (LPN).
- Specialization: Additional certification or training in wound care management.
- Experience: Previous experience in wound care highly preferred.
- Communication Skills: Excellent verbal and written communication skills.
- Compassion: Demonstrated empathy and compassion towards patients and their families.
- Adaptability: Ability to adapt to different settings and work effectively in a mobile care delivery model.
