We are a family of Physicians and Nurse Practitioners dedicated to providing compassionate care to patients with serious illnesses and end-of-life needs.

Job Summary

We are seeking a Visit Physician to join our team. As a Visit Physician, you will provide symptom management and ongoing goals of care discussions with our patients.

Responsibilities
  • Display expertise in providing symptom management and ongoing goals of care discussions with our patients.
  • Possess excellent communication skills with an understanding of working with seriously ill and/or terminal patients and their families.
  • Work closely with the medical director, the team physicians, and the team leaders to provide continuity of care consistent with the hospice philosophy and the patient's wishes.
  • Work effectively with all members of the hospice team.
  • When a physician provides visits in an inpatient setting, call responsibility is included in the rounding responsibility.
  • Complete all documentation within 24 hours of the visit.
Qualifications
  • Maintains an active License and DEA in the state where care is being provided.
