Job Summary

Athol Hospital is seeking a highly skilled and dedicated Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for ensuring the safe and effective use of medications in our hospital setting.

Key Responsibilities
  • Enter medication orders accurately into patient medication profiles in compliance with hospital formulary.
  • Interpret medication orders correctly and maintain accurate and complete patient medication profiles.
  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
  • Consistently perform DUR (Drug Utilization Review) before entering medication orders.
  • Provide double checks as needed on pharmacokinetic consults to detect calculation errors and assess outcome goals.
  • Document interventions in the medical record that advocate IV to PO conversion, dose optimization, dose adjustments, therapeutic duplications, drug-drug interactions, allergy conflicts, and other interventions groups for Antimicrobial Stewardship and other medication therapies.
  • Supervise the manufacture and distribution of medications and IV solutions by the Pharmacy Technicians to ensure medications are delivered to the patient care area staff at the scheduled times of administration.
  • Contact prescribers about questionable orders and provide information on the correct therapy, noting formulary alternatives when indicated.
  • Educate patients and clinicians regarding medications.
  • Collect data for DUR and ADR programs.
  • Prepare chemotherapeutic medications according to the program so that patients receive the correct dose and medication at the scheduled time.
  • Systematically gather and evaluate relevant information and data from the patient interview and/or all the patient information sources.
  • Consistently look at Med Recs for appropriate dosing.
  • Recommend appropriate antibiotic therapy based on culture and susceptibility data or suspected infection.
  • Use appropriate antibiogram to see sensitivity data.
  • Demonstrate an understanding and complete all job functions in accordance with the five patient rights.
  • Follow hospital policy in reporting medication errors and adverse drug reactions.
  • Assist physicians and nurses with medication reconciliation.
  • Follow all policies and procedures related to inventory management and control.
Requirements
  • Bachelor's degree in Pharmacy.
  • Clinical background to help develop programs.
  • Current licensure as a Pharmacist in the state of Massachusetts.
  • Good communication skills to relate with medical and nursing staff.
