Pharmacist - Bi-Campus Pharmacy

We are seeking a highly skilled and dedicated Staff Pharmacist to join our Bi-Campus Pharmacy team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring the safe and effective use of medications.

• Enter medication orders accurately into patient profiles and interpret orders correctly.
• Monitor drug therapy regimens for contraindications, interactions, and appropriateness of drug and dose.
• Provide double checks on pharmacokinetic consults to detect calculation errors and assess outcome goals.
• Document interventions in the medical record to advocate for IV to PO conversion, dose optimization, and other medication therapies.
• Supervise the manufacture and distribution of medications and IV solutions to ensure timely delivery to patient care areas.
• Educate patients and clinicians regarding medications and collect data for DUR and ADR programs.
• Prepare chemotherapeutic medications according to program protocols to ensure accurate dosing and administration.
• Systematically gather and evaluate patient information to ensure appropriate dosing and medication use.
• Recommend appropriate antibiotic therapy based on culture and susceptibility data or suspected infection.
• Follow hospital policy in reporting medication errors and adverse drug reactions.
• Assist physicians and nurses with medication reconciliation and follow all policies and procedures related to inventory management and control.

• B.S. in Pharmacy
• Clinical background to develop programs
• Current MA licensure
• Good communication skills to relate with medical and nursing staff
