About the Role

PennState Health is seeking a highly skilled Pharmacy Technician to join our team in a Medication Reconciliation Specialist role. As a key member of our healthcare team, you will play a critical role in ensuring the accuracy and safety of patient medication lists.

Key Responsibilities
  • Obtain and document accurate medication lists for newly admitted patients to prevent medication duplications, omissions, and errors.
  • Enter medication information into the electronic health record for use by physicians and other healthcare providers.
  • Collaborate with healthcare teams to ensure seamless medication management and patient care.
Requirements
  • High School Diploma or equivalent required.
  • Current Pharmacy Technician certification or currently licensed to practice as a licensed practical nurse by state of employment or holds a multistate LPN license through the interstate Nurse Licensure Compact.
