Job Summary

PennState Health is seeking a highly skilled Pharmacy Technician to join our team in a Medication Reconciliation role. As a key member of our healthcare team, you will be responsible for ensuring the accuracy and completeness of patient medication lists, thereby preventing medication errors and improving patient outcomes.

Key Responsibilities
  • Obtain and document accurate medication lists for newly admitted patients to prevent medication duplications, omissions, and errors.
  • Enter medication information into the electronic health record to facilitate communication among healthcare providers.
  • Collaborate with healthcare teams to resolve medication-related issues and improve patient care.
  • Stay up-to-date with changing medication lists and ensure accurate documentation.
Requirements
  • High School Diploma or equivalent required.
  • Current Pharmacy Technician certification or licensure as a licensed practical nurse by state of employment or multistate LPN license through the interstate Nurse Licensure Compact.
